Evidence you may need to support an LTFT request

What this article is about
This article explains when evidence may be needed for an LTFT request.

Who this is for
Doctors and dentists in training preparing an LTFT application or change request.

What you need to know

  • Some LTFT requests may require supporting evidence.
  • Evidence requirements depend on the reason for the request.
  • Requests linked to disability, ill health, caring responsibilities, welfare, wellbeing, religious commitments, development opportunities or other circumstances may need relevant supporting information.
  • You should only submit information that is relevant to the request.
  • Missing or unclear evidence may delay the application or mean it cannot be approved.
  • If evidence is required but not provided, the application may be rejected.
  • Sensitive information should be shared only through the correct route.

What to do

  1. Check the LTFT guidance and application form.
  2. Identify whether evidence is required for your reason for applying.
  3. Gather current and relevant evidence before submitting the application.
  4. Submit only the evidence needed to support the request.
  5. Explain if evidence is not available and ask what alternative information may be accepted.
  6. Keep a copy of anything you submit.
  7. Respond promptly if further information is requested.

If this does not work
If your evidence is incomplete or does not support the request, you may be asked to provide further information. If evidence is still not provided where required, the application may not be approved.

Who to contact
Use the PGMDE Support Portal for LTFT evidence queries. Contact your Training Programme Director or educational supervisor if you need to discuss how the request may affect your training.
